Abstract

This article discusses post-traumatic exogenous endophthalmitis as an ophthalmological emergency that can potentially lead to permanent vision loss if not treated promptly and appropriately. The purpose of this study was to analyze the appropriateness of clinical manifestations, risk factors, supporting examination findings, and case management of endophthalmitis based on theoretical studies and a case report of a 44-year-old farmer who experienced eye trauma due to mud infiltration in a rice paddy. The method used was a case study with a descriptive-analytical approach through the collection of patient clinical data, ophthalmological examination, supporting examinations, and a review of relevant literature on endophthalmitis. Data were analyzed by comparing the patient's clinical condition to current theoretical concepts, pathophysiology, diagnosis, and management of endophthalmitis. The results showed that the patient experienced typical symptoms of endophthalmitis, including decreased visual acuity, severe eye pain, redness, photophobia, eyelid edema, and hypopyon. Ultrasonographic examination revealed a visual acuity of 1/300 in the left eye, corneal erosion, and an inhomogeneous hyperechoic lesion in the vitreous body, indicating severe Vitritis. A history of trauma due to exposure to rice paddy mud supports the diagnosis of post-traumatic exogenous endophthalmitis. Initial treatment with antibiotics, antifungals, cycloplegics, and lubricants was administered, but definitive therapy requires intravitreal injection and consideration of early vitrectomy to preserve visual function.
