Ovarian cysts are a gynecological disorder that can cause reproductive disorders and acute complications such as ovarian torsion. This condition is often found in women of reproductive age and can be associated with primary infertility. This study aims to describe the clinical course of a patient with a right ovarian endometriosis cyst accompanied by primary infertility that progressed to ovarian torsion. The study used a descriptive method in the form of a case report of a 38-year-old woman who complained of sudden worsening right lower abdominal pain. Data were obtained through anamnesis, physical examination, laboratory tests, ultrasonography, surgery, and histopathology. The examination results showed a cystic mass of the right ovary measuring ± 8 × 9 cm with a diagnosis of right ovarian cyst torsion and primary infertility. Histopathology examination showed a right ovarian endometriosis cyst. The patient underwent a right partial oophorectomy with improved postoperative condition. This case illustrates that ovarian endometriomas can be associated with fertility disorders and develop into acute complications requiring immediate operative treatment in women of reproductive age.
