Abstract

This study addresses the high chicken mortality rate in the rooster chicken production process at a small-scale (UMKM) poultry farm in Kalongan Village, Prigen, Pasuruan, which partners with PT Japfa Comfeed. The study aims to measure the defect rate, identify the root causes, and propose improvements using the Six Sigma method with the DMAIC (Define, Measure, Analyze, Improve, Control) approach. A descriptive quantitative approach was applied using production data from an initial population of 13,000 day-old chicks (DOC) over a 10-week rearing period (October–December 2025). The results showed a cumulative defect of 496 birds (401 mortalities, 52 underweight birds below 1 kg, and 43 culled birds), yielding a defect rate of 3.8%, a DPMO of 12,718, and a long-term process capability of 3.74 sigma. Fishbone analysis using the 6M framework identified the dominant root causes as worker fatigue, an unstable manual LPG brooder, inconsistent SOP compliance, wet rice-husk bedding, manual recording, and environmental threats such as flooding and predator attacks. Improvement proposals were formulated using the 5W+1H matrix and locked through control standardization, which are projected to reduce the defect rate and increase the sigma level.
