Abstract

Background: Auditory hallucinations are one of the most common symptoms experienced by patients with mental disorders and can interfere with thinking, behavior, and social interaction. Positive distraction therapy using the Iqibla Smart digital dhikr device is one of the non-pharmacological therapies that can help patients control hallucinations by shifting attention from internal stimuli toward a structured spiritual activity. Objective: To describe the application of positive distraction therapy using the Iqibla Smart device in patients with auditory hallucinations at dr. Arif Zainudin Regional Psychiatric Hospital, Surakarta. Methods: This study used a case study design with a nursing care approach involving two patients diagnosed with schizophrenia who experienced auditory hallucinations. The intervention was carried out for three consecutive days using dhikr therapy with the Iqibla Smart device for approximately 10–15 minutes after each of the five daily prayers. Data were collected through interviews, behavioral observation, and nursing documentation. Results: After the intervention, both patients showed a decrease in the frequency and intensity of hallucinations. The first patient's hallucination frequency decreased from three times per night to once a day, while the second patient's hallucinations became infrequent. Both patients were able to recognize the onset of hallucinations, apply the rebuking technique, perform dhikr independently using the device, and showed improved emotional calmness and social interaction. Conclusion: Positive distraction therapy using the Iqibla Smart device can help patients control auditory hallucinations by redirecting attention from internal stimuli to a more adaptive spiritual activity, and can be used as a complementary non-pharmacological intervention alongside standard psychiatric nursing care.
