Abstract

Hepatobiliary resection remains the primary surgical option for managing both benign and malignant diseases of the liver and biliary tract. Although conventional open surgery has long been the standard approach, minimally invasive or laparoscopic techniques continue to evolve and are increasingly applied to complex cases. However, debate persists regarding their long-term efficacy and oncological safety compared to open surgery. This study aimed to comprehensively evaluate and compare the short- and long-term outcomes of both surgical approaches. Through a systematic review and meta-analysis of relevant studies, perioperative data including intraoperative blood loss, length of hospital stay, complication rates, as well as oncological outcomes such as overall survival and disease-free survival were synthesized. The analysis revealed that the laparoscopic approach offers significant advantages in short-term outcomes, including reduced blood loss, faster functional recovery, and lower postoperative complication rates. Regarding long-term outcomes, the efficacy and oncological safety of laparoscopic techniques proved comparable to open procedures. Consequently, minimally invasive hepatobiliary resection represents a safe and effective alternative that warrants consideration as a standard clinical management strategy in digestive surgery.
