Abstract

Anemia during pregnancy remains a maternal health problem that requires attention, particularly among third-trimester multigravida women who experience increased iron requirements approaching delivery. In Bangkalan Regency, 2,505 cases of anemia were recorded among 19,305 pregnant women (12.98%). This case report aims to describe the assessment, midwifery analysis, management, and clinical progress of a third-trimester multigravida woman with anemia at Midwifery Independent Practice V, Tanah Merah, Bangkalan. A descriptive design with a single case report approach was applied to Mrs. L, aged 31 years, G2P1A0, through three home visits conducted on June 19, June 25, and July 2, 2026. Data were obtained through interviews, observations, physical and obstetric examinations, hemoglobin measurement, the Maternal and Child Health (MCH) Handbook, and SOAP documentation. At the first visit, the mother had no major complaints but presented with pale conjunctiva, a hemoglobin level of 10.8 g/dL, irregular iron tablet consumption, and suboptimal dietary patterns. Management included education on anemia and nutrition, appropriate consumption of iron and calcium tablets, increased intake of iron and vitamin C, moringa leaf tea as complementary therapy, adherence monitoring, management of third-trimester discomfort, birth planning and complication prevention (P4K), and family involvement. At the third visit, hemoglobin increased to 11.6 g/dL, the conjunctiva appeared pink, adherence improved, and physiological complaints decreased. Clinical improvement occurred following continuous care; however, the single-case design and concurrent interventions prevent determination of the contribution of each intervention. Continued monitoring of hemoglobin, iron tablet adherence, dietary patterns, and antenatal visits is recommended through the postpartum period.
